I also agree with separating first from subsequent recurrences.  The control arm is likely to be different, as most will have already been exposed to taxanes and PLD by second recurrence.  I support preference for carbo/caelyx for first recurrence but there are several suitable options for beyond first recurrence, including in my opinion, single agent carboplatin.  We don’t actually have evidence that combination is better beyond first recurrence.
